Use the form below to request a search for a death certificate from our archives. All requests for death certificates must include a name and year of death. City Archives & Special Collections only holds copies of death certificates from 1804-1915.
Fee: $2 per name searched and up to 1 page copied

For certified copies of Death Certificates 50 years or older, and please contact the State Archives. For Death Certificates from 1915 to 50 years ago please also contact the State Archives.
For Death Certificates from the last 50 years, please contact the Vital Records Registry.
